Looking To Get Equipment For Your Child/family?
There Are 2 Ways To Get Set Up:

Option 1
Get a Referral from one of our Referring Agencies/Organizations

  • See an agency you work with? Ask them to connect you with our services and they will refer you to us!

Option 2
Individuals/Families can self-refer using one of the following verification methods:

  1. City of Edmonton Leisure Access Pass (LAP)
  2. Alberta Works Health Benefits Card – Link example
    (*not the same as AB Health care Card*)

  3. AISH

Once completed, you MUST attach one of the documents listed above.

NEXT STEPS:

1. Wait For Approval: After you have submitted your application, please wait a minimum of 48 hours after submission for a reply.

2. Book An Appointment: If you have been approved, we will contact you to book an appointment for fitting. *Appointments must be booked and confirmed in advance*.

  • Once booked, an email & text will be sent to you with your appointment date/time.
  • Note: Kids MUST be present for Sport Equipment appointments to ensure accurate fitting of equipment.

3. Get Fitted: Please arrive no more than 10 minutes early for your appointment. We will do our best to accommodate late arrivals, but may have to reschedule out of respect for other client appointments.

  • You will receive a reminder 48 hours prior to your appointment.
  • Families are allotted a specific time slot for their appointment. If you are running late, or need to make a change, please phone and notify Sport Central as soon as possible to 780-477-1166.
  • If you are exchanging equipment previously received from Sport Central, please bring it to your appointment.

4. Help Us Help More Kids: After your visit, you will receive an email to complete a short survey of your experience. We would be grateful for your participation as this helps us better serve the kids in our community.
